We’re building a mobile vaccination card that allows any clinic to enter official shot information, quickly and painlessly. Records are synced in real-time to patients’ mobile devices. Patients simply provide a unique ID and any medical facility, camp, school, etc. can instantly view and download the official record.

Target Audience: Women between 18 and 55 who live in the United States. Have 1+ children. Use mobile phones/ipads. Understand technology. Like to travel.

  1. 1. I found it challenging to sign up as a patient. I downloaded the site and am signed in to access the app. I can verify that I am signed in as a user and that my records are accessible. Not sure how they are accessible when no medical information has been provided. I am finding the initial opening of the site confusing and am unsure what to do next.

    2. It would be helpful if there was a welcome new member section with clear instructions of what to do next. I hit the arrows on the area with my name and marked records in access. This verifies that you do not have any current records and I am not sure what to do next to provide the information. This is especially perplexing for something like medical records which has so much security associated with it. Need to have easy step to categories and give instructions on how people can locate the medical information they need it they are not sure of it.

    3. Sign up fairly smooth. Looks like it will be very time consuming to fill in all the information. Setting up password and user name was not problem.

    4. Looking further into the app I have located a page that appears to be for medical records. I am test filling it out. While checking out the categories surgical history etc I hit a button by accident that brought out the previous page of the websites.I am not impressed with the icons on the personal page that breaks out the categories. It is ok to use a graphic but there should be wording with it so you don’t have to search and memorize the symbols. Some are self evident and others are not. Under medication history it would be better to say name of medication rather than just name. People will confuse this with their own name. The medication page can be greatly improved. It could have recognition so that once a few letters are filled in it drops down a choice of drugs. Many people do not know how to spell the names of their medications. It would also be good to have a distinction between generic and name brand drugs. It would also be good to have a drop down dose of each medication since the strengths are limited. I have no idea what route means. I do not know if refill means the date it can be refilled or when you need a new subscription.Medical history should have subcategories. Do you want only major conditions? Surgical history is clear but will it be a repeat of medical history? Immunization history is clear. Allergy history could include the symptoms of the allergy. Family history could be more specific and broken down by how far back in family history you want to go.

    5. I assume family history is done under people but am not sure. Could use more guidelines. It is easy to add the information but I am not confident I am doing it in the correct area. This site needs to add a lot of user friendly icons or written directions. Would be nice to have drop down menus and word identification. I am having trouble navigating the site. It is hard to get from the main screen back into the categories like vaccines. I also Have no idea what information would be under the social area. It is not hard to add family members although my spouse has a complicated medical history and not sure this reflects the extent of it.

    It’s a very simplistic design. However there is no instructions or directions on what to do when first opening the app.

    The installation and setup process was very straightforward. I didn’t understand why you had to create a password when first signing up but then had to switch it to a pin after logging into the app.

    It was a little difficult to find where the “Records Request” was located and then once I found it I did not know how to search or add a practice. Once I finally found it the rest was pretty straightforward. “Records Request” allows you to request your records from your provider. The problem was that not a lot of hospitals or clinics in my area seemed to be signed up for this service that it had to be entered manually.

    I would make a note about it in the “Notes” section and then put it under “Studies”. It sounds like you might be able to request it under “Studies” but because of the lack of instructions I am unsure.

    This is not a very easy process and is very tedious. The app has a good concept but lacks a sense of direction and instruction. Storing vaccine information on this app could be useful to access for my family and know what is up to date but the app needs to become more user friendly.

    I downloaded the application, and registered my account through the email address: verduistering@gmail.com.

    2) Look around the app – what is your initial impression of what you see?
    As soon as I entered the application, it asked me to create a “pin code”, it made me realize that the application is very secure. The appearance of the application seem very nice and it has very nice and light colours, it makes the application seem more userfriendly. The application also seemed very easy to use.

    3) After you downloaded the app, was the install/launch/signup process straightforward? Did you find anything confusing?
    The installation was very easy, but the signup process was a bit confusing, the blank spaces where I have to put my information, seem very tiny in size and it seem difficult to put in my details there. After registration is complete, it tells me to LogIn, but the LogIn screen asks me for my Username, but in the registration process, it didn’t ask me to put in a username. Instead I had to put in my email address where it asked me the username in the Login screen.

    I went to “Records Request” and searched for the name of my hospital. It didn’t show up, but then I placed in my city details, and left the information for hospital name blank. My hospital automatically showed up, it seem very nice and fast. It seemed quite straight forward, “Record Request” allows me to request my records to this application through my hospital records.

    I would first click on the bar on the top right, and then choose “PERSONAL”, after choosing “PERSONAL”, I selected the “IMMUNIZATION HISTORY” and clicked on the plus icon. The details it asked were a bit confusing as it asked for certain details such as “Provider name”, “Specialty” etc. But overall, it was quite easy and seemed userfriendly.

    This was the easiest process in the application, all I would have to do was to choose the “FAMILY HISTORY” option from the “PERSONAL” area and fill up the details. Storing vaccine information was easy but a bit confusing as the details which I had to fill up (such as Provider Name, and Specialty) seem not necessary

    First thing when I logged into my account, I am required to create a Pin which gives a sense of how much security they are willing to put onto this app to prevent others from bypassing other’s personal/private information. Afterwards, I am brought to the menu screen which looks clean and easy to navigate through. A nice simple and colorful layout!

    The installation was quick, launching was smooth, and the signup process was very straightforward. Only thing I want to bring up was that the text was a bit tiny which might be difficult for others to see, unless that’s my phone’s size – iPhone 5s.

    I tapped on the menu on the top left corner to find the “Records Request” option. I didn’t see any search option to search for my hospital/clinic/practice but eventually figured out I had to press the + sign on the top right corner. It wasn’t completely straightforward because of the lack of instructions/guidance. There could be a tutorial in the beginning to help. “Records Request” allows me to search and add my hospital onto the list after saying the city, state, and the name of the hospital.

    I would politely ask them to have my vaccine record to be inputted through the app and show them the liability of the app, explaining to them that it is safe and used by thousands of other people.

    First of all, I didn’t know where to go to add my family’s information. This wasn’t an easy and smooth process with nothing that indicates where I should go to start. So I looked through the menu assuming it should be in “Personal.” Even after going to “Personal,” I had to look for the option to add my family’s information which is a yellow icon of 3 people. This picture can be misleading without any words to say that it’s where you add your family’s information. I believe the ease of storing vaccine information on the app is that you can have it with you wherever and whenever you are. Nowadays, schools and jobs ask for your vaccination shots to check and make sure you are vaccinated and allowed to enter them.

    I downloaded and installed the application on my iPhone 5, the process was very easy. Then I registered as a patient with the email address of paulaislai99@hotmail.com.

    There is nothing to see in the application. My initial impression is that the entire application seem very empty and without any specific information in the application. There is no information on the homepage about the application. Usually other applications have information about how to use the application right after I log in.

    The signup process was very straightforward. There was no specific problem in the install/launch/signup. In the signup process I was a bit confused when it did not allow me to sign up, and I didn’t know that I had to confirm my email address. Apart from that everything was very easy.

    I went to the “Records Request” by going to the sidebar, I then entered my city information and the hospital information. I was easily able to discover my hospital: Corpus Christi Medical Center in Texas.
    It was very easy to find my hospital in the data, and the procedure was straightforward. According to me, I think that the “Records Request” allows me to request for my records in that hospital.

    I would have to go to “Personal” option from the sidebar. Then I would have to click the syringe button and later on click the “+” icon on the top right. After that, all I would have to do is to fill up the details asked on the “Immunization History”

    The process was very easy, all I had to do was go back and click on the “yellow” family icon in the “Personal” section of the page, and fill up the details about the family.
    I think that the ease of storing vaccine information on this application is very easy, as all I had to do was to fill up short amounts of details.
